Job Summary
We are seeking a highly skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing in-home assessments and overall care-management to a caseload of frail older adults.
About the Role
The ideal candidate will have a background in home care or hospice and a passion for managing geriatric care-planning in a team environment. The RN Case Manager will assess the need for, and coordinate the delivery of, nursing and home care services.
Responsibilities
- Conduct in-home assessments of patients
- Coordinate the delivery of nursing and home care services
- Attend interdisciplinary meetings and care-planning meetings
- Document patient information and progress
Requirements
- Graduate of an accredited nursing school
- At least one year of experience working with a frail elderly population
- Licensed Registered Nurse in Pennsylvania
- Valid Driver's License
