Nursing Care Coordinator

Job Summary: We are seeking an experienced On-Call Registered Nurse to join our team at Community LIFE. The successful candidate will have a strong background in gerontological nursing and excellent communication skills. As an On-Call RN, you will be responsible for delivering high-quality care and support to older adults in their homes.

Key Responsibilities:

• Deliver high-quality care and support to older adults in their homes
• Work closely with healthcare providers and caregivers to develop and implement care plans
• Evaluate participants for episodic illness after business hours
• Document care provided and changes in participant condition in a timely manner

Requirements:

• Graduated from an accredited nursing school
• Minimum one year of experience working with frail or elderly populations, preferably in long-term care, home care, or community health
• Registered Nurse licensed in the state of Pennsylvania
• Possess a valid PA Driver's License
