Job Description
As a RN Case Manager, you will work closely with an interdisciplinary team to provide comprehensive care to older adults in their homes. You will assess the needs of patients, develop care plans, and coordinate services to meet those needs.

Responsibilities
- Assess patient needs and develop care plans
- Coordinate services to meet patient needs
- Attend interdisciplinary meetings and care-planning meetings
- Document patient information and progress
Requirements
- Graduate of an accredited nursing school
- At least one year of experience working with a frail elderly population
- Licensed Registered Nurse in Pennsylvania
- Valid Driver's License
