Responsibilities
- Maintain a caseload of Community LIFE participants and provide psycho-social assessments and interventions related to their maintenance in the community.
- Perform utilization case management for all acute inpatient admissions and transfers.
- Work closely with healthcare professionals to develop and implement comprehensive care plans.
- Provide education and support to participants and their families on various topics related to aging and health.
Requirements
- Master's Degree in Social Work (MSW) from an accredited school of social work.
- Minimum one year experience working with the frail or elderly population, preferably in a hospital, long-term care, or community setting.
- Excellent verbal and written communication skills, with the ability to work effectively with people from diverse backgrounds.
- Able to work independently and be self-directed, with good problem-solving skills and a compassionate attitude towards older adults.
