Job Description
We are seeking a skilled Social Worker to join our team, who will be responsible for maintaining a caseload of Community LIFE participants and providing psycho-social assessments and interventions related to their maintenance in the community. The ideal candidate will have a Master's Degree in Social Work (MSW) from an accredited school of social work, and at least one year experience working with the frail or elderly population.
Key Responsibilities
- Maintain a caseload of Community LIFE participants.
- Provide psycho-social assessments and interventions related to community maintenance.
- Perform utilization case management for all acute inpatient admissions and transfers.
- Collaborate with healthcare professionals to develop comprehensive care plans.
Requirements
- Master's Degree in Social Work (MSW) from an accredited school of social work.
- Minimum one year experience working with the frail or elderly population.
- Excellent communication skills, with the ability to work effectively with people from diverse backgrounds.
