What are the responsibilities and job description for the RN Supervisor - Part Time position at Community Living Alliance?
Purpose
This role works across all programs in the Community Living Alliance Care Solution's team to provide direct oversight and supervision to Personal Care Workers. The role is key in ensuring our clients receive quality service and have the opportunity to live with health, independence and dignity at home. Someone in this role will be working part-time for 4.5 hours daily to ensure their workload is properly balance throughout the week while enjoying harmony between your workday and personal and family needs.
Primary Duties:
- Conduct assessment visits with clients in accordance with CLA policies and State Regulations and Rules.
- Coordinates and manages the care solution needs of clients by providing training for caregivers.
- Evaluates the need for services and makes referrals to other services as appropriate.
- Develops a plan of care for the client, giving full consideration to the client's preferences for service arrangements and choice of caregivers. Secures written orders from the client's physician, if applicable.
- Provides administrative, coordination and direct service support for clients.
- Provides coordination and monitoring for clients served by Supported Living Providers.
- Validate caregivers time and service through electronic visit verification system (EVV).
- Supervise, mentor and develop caregivers on RN's caseload upon hire and throughout duration of providing care

Minimum Education/Experience and Requirements:
- Associate's Degree in Nursing, required. Bachelor's Degree in Nursing, preferred.
- Current licensure as a Registered Nurse under s. 441.06, Wisconsin Statues.
- 1 year supervisory experience in personal care services, community based home care (non-skilled nursing) or related program, preferred.
- 3 years nursing experience in personal care services, or related program, preferred
- Experience working with people with disabilities and elderly, preferred
- Valid driver's license, reliable transportation, and current auto insurance to travel throughout Dane County, required.
- Must be able to lift up to 35 pounds, required.
- Work around environmental hazards such as odors, second-hand smoke, animals, cleaning supplies, adverse living conditions, etc. Required
- Satisfactory Results of a WI Caregiver Background Check, required.
