RATE OF PAY: $21-23 $1 weekend differential rate from Sat 8 am to Sun 11 pm
SCHEDULE: 10AM-6PM Monday-Friday
SUMMARY
Provide residential routine support, skill building, and companionship in a home setting and the community to enable the individuals with intellectual and developmental disabilities to pursue their life ambitions and goals and to maintain their health and safety.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. All persons in this position must be capable of performing these functions, in adherence to all CLA policies and procedures.
Provides input for the development of the annual Individual Service Plan (ISP) for each individual supported, writing, typing or proof-reading documents as needed.
Implements the particular requirements of the ISP on a daily basis.
Assists Home Manager in implementing or ensuring the implementation of the particular requirements of the ISP on a daily basis. In the ICF, ensures compliance with all pertinent federal regulations applicable to community ICFs.
Maintains all required documentation in logs and records, including monthly and quarterly reports.
Ensures that individuals supported have personal choices in all aspects of their daily life, and that the individuals are empowered to pursue their choices.
Provides for the safety of the individuals supported by monitoring at all times per individual plan, and for the security of the home and by ensuring the home is clean and free from hazards, including removal of fallen tree limbs, leaves, snow, ice, objects on stairs, electrical cords, spills or other conditions that may present safety issues.
Establishes and maintains cooperative and professional relationships other CLA staff, with the families or guardians of the individuals supported, and appropriate interested outside parties.
Manages and documents emergency situations and notifies appropriate staff.
Schedules, implements, and follows-up on medical and dental appointments/referrals, as required.
Provides training, assists and supports the individuals supported in all activities of daily living (ADLs) according to their specific support needs. These areas will include hygiene, wellness matters, housekeeping, communications, socialization, appropriate use of leisure time, shopping and money management, use of community resources, and other activities of daily living, as necessary.
Assist, support, oversee, model and/or train individuals on personal care issues including bathing, toileting, ostomy care, catheters, diabetes monitoring, and other related functions.
Reports need for painting and building or equipment repair of the home to the Administrative Office in accordance with established procedures.
Requisitions needed materials such as office supplies, stamps, latex gloves, etc., in accordance with established procedures.
Ensures individuals have safe, appropriate and reliable transportation to medical appointments, community activities, emergency evacuations, day programs, etc. as necessary and determined through the ISP. This transportation may be provided by staff utilizing an agency vehicle or their personal vehicle, if authorized, or it may be provided through public transportation, car services, or Medicaid transportation as appropriate and necessary.
Implements and documents the criteria of behavioral support plans for individuals supported, and participates in the development of such plans.
Encourages individuals supported to participate in social, educational, and recreational activities.
Attends relevant and scheduled staff meetings.
Performs other duties as directed.
SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.
QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE
At least one year of documented experience working directly with individuals who have intellectual disabilities or other developmental disabilities, and one of the following credentials: (i) a doctor of medicine or osteopathy licensed in Virginia, (ii) a registered nurse licensed in Virginia, or (iii) completion of at least a bachelor's degree in a human services field, including, but not limited to sociology, social work, special education, rehabilitation counseling, or psychology.
LANGUAGE SKILLS
Ability to speak, read and interpret documents in English, such as safety rules, operating and maintenance instructions, and procedure and policy manuals. Ability to write routine reports and correspondence. Ability to speak effectively with family groups of the individuals supported, other interested parties, or employees of CLA.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited guide rules exist. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
It is preferred that the employee has a valid driver’s license and clean driving record in order to have and use his or her privately owned vehicle while on duty. The vehicle must be legally licensed and registered and properly insured according to the laws of the state of the employee’s residence. The vehicle must be maintained for safe highway operation. If the employee is unable to initially drive he/she will take the necessary steps (i.e. DMV Driver’s Improvement course) or once reaches the required 3 years driving experiences, to be approved to drive while on duty.
The employee is required to maintain CPR/First Aid certification and to receive required training and certification in such other areas as are necessary for CLA licensure.
