We are seeking qualified applicants for a Service Coordinator / Case Manager position, who will work with adults / children with Intellectual and / or Developmental Disabilities (I / DD) who are eligible for case management services.
Responsibilities :
- Provide case management services for adults / children with I / DD and their families / support team
- Assist individuals, families / support team in accessing services and resources available
- Developing, coordinating, and monitoring of Individual Support Plans (ISPs)
- Work with partnering agencies, community organizations, school / educations programs, etc. to provide comprehensive service coordination to supported individuals
- Serve as an advocate / liaison to assist individuals in obtaining and accessing necessary services including obtaining referrals or finding resources in the community
Qualifications :
Bachelor's degree in behavioral science, social science, or a closely related field; or
