Community Living Connections (CLC) is seeking passionate individuals to join our team as Supported Living Staff , providing essential, person-centered support to individuals with developmental disabilities.
Whether you're an experienced caregiver or new to the field, we will provide the training you need to succeed!
Location : Madison West Side
PT w / Two Shifts Available :
- 12 PM to 8 PM Weekends
- 3 PM to 5 PM Mon-Thurs

Key Responsibilities :
Person-Centered Support : Implement individualized care plans tailored to the unique needs and goals of each person you assist.
This includes fostering personal growth, life skills, and community integration.
- Daily Living Assistance : Provide support with household tasks such as cooking, cleaning, and organizing, while encouraging independence in these activities.
- Health & Wellness : Help with personal care routines like hygiene, grooming, and promoting healthy living habits, ensuring a safe and clean environment.
- Community Engagement : Support individuals in exploring and utilizing community resources, attending appointments, and participating in social activities.
- Financial & Transportation Support : Help with budgeting, grocery shopping, and transportation needs based on each individual's schedule.
- Communication & Collaboration : Partner with families, managers, and external professionals to provide high-quality care.
What We're Looking For :
- A desire to support individuals with developmental disabilities.
- Patience, empathy, and excellent communication skills.
- Ability to work independently and as part of a team.
- Willingness to participate in ongoing training and professional development.
- Flexibility to work varying shifts, including weekends and evenings.
- A valid WI driver's license, access to a reliable vehicle, and proof of vehicle insurance that meets CLC's standards.
Qualifications :
- High School Diploma or equivalent.
- No prior experience necessary-we provide all required training.
- Ability to maintain a valid driver's license and meet driving record standards.
Physical Requirements :
- Ability to lift up to 40 lbs. occasionally.
- Ability to stand, walk, bend, and assist individuals with daily activities.

A criminal background check is required. Prior to the background check being conducted, the applicant will be required to
complete disclosure and authorization forms authorizing the Company to conduct
specific background checks. This authorization will be made in either electronic or
written form and will remain valid throughout the employee's employment with the
Company, if hired as allowed by applicable law.
Background checks will be kept confidential and will only be shared with individuals who

Reports will be retained in accordance with the Company's document retention
