As a Hospice RN Case Manager, you will play a vital role in establishing meaningful connections with patients, their families, and your colleagues while providing expert care. You can expect:

As the RN Case Manager, you will assume full nursing responsibility for delivering high-quality patient care, continuously evaluating your performance, and making adjustments to enhance productivity and quality of care.
- Makes initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral
- Identifies patients' and families' physical, psychosocial, and environmental needs, reassessing as needed, no less than every 15 days
- Develops and coordinates patient care plans while maintaining accurate clinical notes regarding the patient's condition
- Documents problems, goals, interventions, and patient/family response to hospice care
- Collaborates with patients, families, physicians, and other healthcare professionals to provide daily patient and family care
Our ideal candidate possesses one year of clinical experience and holds current RN licensure in the state of practice, along with CPR Certification and a valid driver's license.
