We are seeking a skilled and compassionate RN to join our hospice team as a Case Manager.

As an RN Case Manager, you will be responsible for delivering high-quality care to patients in their end-of-life journey.
- You will develop trusting relationships with patients and families, providing expert end-of-life care.
- You will work collaboratively with the interdisciplinary team to ensure seamless patient care.
Job Responsibilities:
- Makes initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
- Identifies patient's physical, psychosocial, and environmental needs, reassessing every 15 days.
- Develops and coordinates individualized plans of care, maintaining accurate clinical notes.
- Collaborates with patient, family, attending physician, and IDG members to deliver comprehensive care.
Requirements:
- 1 year of clinical experience.
- Current RN licensure in the state of practice and 1 year of clinical experience.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to reliable transportation or public transportation.
