What are the responsibilities and job description for the Human Services Specialist position at Community Medical Group?
Summary
Assist clients/visitors/patientswith the completion and submission of the ACCESS (Automated Community Connection to Economic Self-Sufficiency) applications for Food Stamps, Medical Assistance, and Temporary Cash Assistance and Medicare Savings Program. Provide access and referrals for other community agencies.
Duties and Responsibilities
1. Educates visitors/clients/patients customers of the role of a social services agent vs. the DCF role and the application process.
2. Completes the DCF (Department of Children and Families) and Community Medical Group consent forms on-line and obtain the customer’s signature on the electronic signature pad.
3. Explains the advantages of becoming a member of Community Medical Group to our customers.
4. Completes the ACCESS applications for our customers based on the information they provide and submit to the Office of Economic Self-Sufficiency for the client/patient, as assigned.
5. Assists clients in creating a “My ACCESS Account”
6. Follows up daily on the ACCESS application until the Office of Economic Self-Sufficiency determines their eligibility.
7. Contacts visitors and patients and inform them of the determination made on their application by the Office of Economic Self-Sufficiency.
8. Informs customers who are determined eligible for Medicaid, that they have the option to select a health plan of their choice and Community Medical Group as their primary care physician.
9. Assists customers with the health plan selection process.
10. Documents customer demographics and activities that take place during the application process such as health plan selection and enrollment with CMG software.
11. Assists clients/visitors in using the courtesy phone room as necessary.
12. Completes all applications and handle all confidential information in an honest, ethical and professional manner.
13. Ensures that all HIPAA standards are in compliance.
14. Ensures accurate account of the information provided by the client/visitor. Communication with the client/visitor should not be leading them to any specific response.
15. Performs other duties as assigned
Qualifications
Education/Experience:
1. Must have High school diploma or equivalent.
2. Previous experience in customer service.
3. Previous experience with DCF or other government assistance programs preferred.
4. MS Office programs and client software experience.
5. Experience in Data Entry
