What are the responsibilities and job description for the CARDIAC REHAB RN position at COMMUNITY MEMORIAL HEALTHCARE, INC?
The current position is scheduled Tuesday, Wednesday, and Thursdays from 07:00 to 15:30.
Job Summary: Provide direct patient care to Phase II Outpatient Cardiac Rehabilitation patients. Initiates assessment and care plan based on the patient’s presenting problems and the provider’s medical diagnosis. Must possess a strong knowledge of arrhythmia interpretation and cardio-pulmonary risk factor education. Collaborates with other health care team members regarding treatment planning. Keeps providers and health care team members informed of patient’s condition and rehab status.
Duties and Responsibilities:
Provide a therapy program that includes baseline patient assessment.
Communicate effectively between patient, patient's family, providers and other staff members of the health care team in evaluation of care to ensure continuity of care.
Collects, evaluates, analyzes, documents and prioritizes pertinent patient data, using the full range of patient needs - physical, psycho social, spiritual, education and discharge needs.
Follows provider order and reviews patient information to assess patient condition and determine requirements for treatment and/or procedure such as type and duration of exercise and education.
Assesses tolerance of activity levels, collaborates with other team members and makes recommendations/ referrals regarding appropriateness of return to work or activities of daily living.
Completes ongoing patient assessments as determined by patient condition. Re-prioritizes care and sets new goals based on new information.
Involves and provides information to family and/or significant individuals in assessment activities and patient progress and condition.
Performs initial assessments and initiates communication with health care team members regarding treatment planning; i.e. exercise regimen, education, etc.
Performs daily charting, department billing and chart audit adjustments per hospital standards.
Collaborates with staff and appropriate resource persons to formulate and update treatment plans to work towards positive patient outcomes.
Shall be competent in all exercise equipment usage in Cardiopulmonary Rehab area prior to instructing patient use.
Assist with scheduling of therapy and education courses with the patient and the multidisciplinary team.
Schedule and assist with nuclear stress testing. Collaborate tests with providers, respiratory therapy and nuclear medicine.
Assist with budgeting for the department. Obtaining necessary equipment and supplies considering fiscal responsibility for the organization.
