Job Summary:

This temporary position will work for no more than 89 days. Interviews patients/residents (Swing Bed) and relatives to obtain social history relevant to medical problems and planning. Helps with environmental difficulties/barriers which interfere with obtaining maximum health benefits from medical care; identifies and coordinates discharge planning and related medical social service work to hospitalized patients/residents (Swing Bed) of all ages (neonates, children, adolescent, adults, geriatric) and their families from admission to discharge. May determine the appropriateness of medical necessity of admission, continued stay or use of ancillary services through record review.

Requirements

• Graduate of an accredited college for Social Work; Bachelor’s degree required; Master’s degree in Social Work desirable.
• Licensed by the State of Minnesota Board of Social Work.
• Two (2) years of experience as a Social Worker in long term care desired.

Wage

Depending on your work experience, you could earn a wage from $28.71 to $40.19 per hour.
