What are the responsibilities and job description for the HIM/ROI Clerk position at Community Memorial Hospital?
Duties and Responsibilities:
- Processes incoming requests for release of medical record information in a timely manner and consistently verifies validity of request and authorization.
- Properly receives, sorts and logs departmental mail on a daily basis. Consistently ensures all correspondence is stamped with the proper received date.
- Diligently reviews requests for proper authorization, expired releases, correlation on dates of service.
- Competently reviews medical record for identification of specific protected information (including HIV related information and substance abuse documentation).
- Proactively handles the receipt and distribution of subpoenas. Accurately validates the patient, dates of service, and overall appropriateness of the subpoena.
- Promptly print or fax information from the Lifetime Clinical Record and/or MPF and follows through to completion in an organized fashion.
- Effectively coordinates and assists HIM Team Lead with various projects performed on a daily, weekly, and monthly basis. These may include retrieval of payment for over-due invoices, removal and cancellation in the ROI tracking system requests without response to pre-payments, and burning CD 's for third party payers.
- Courteously assists customers with the functionality of the patient portal (MyHealthChart). Proactively troubleshoots to resolve issues and reset passwords or initiates new access/key codes, as necessary.
- Competently preps, scans, and indexes chart’s within DCS and MPF. Proactively assists with the analysis of records and coverage on DNFB, as assigned.
- Consistently maintains the highest level of confidentiality with all departmental information.
- Retrieves medical records for outpatient services which include, but are not limited to, Community Memorial Hospital Family Health Centers.
- Assumes other duties as assigned by department leadership
Job Requirements:
- High School Diploma or equivalent
- Two (2) or more years of experience in an HIM environment
- Certification from an Accredited HIM program (preferred)
- RHIT/RHIA (eligibility in progress) If eligible, certification must be established within one (1) year of graduating from the respective program
