CMHA is looking to hire a Part-Time 20hrs per week Peer Navigator. This position will be located in New Britain, CT 06051. (Mon - Thur 9:00am - 2:00pm).
Compensation: $18.00 minimum to $22.00 maximum per hour.
Starting salary will be determined based on relevant years of experience.
Position Summary: The Peer Navigator provides initial and ongoing outreach and support to potential and current clients. They support, teach, advocate and empower individuals with behavioral health and substance use disorders, which includes developing person-centered recovery goals. Liaison with local emergency rooms, detox centers and treatment facilities. They engage in traditional and recovery-oriented systems navigation, coordination of services, skill building and eliminating any barriers to care. The goal is to empower and educate clients to rediscover their ability to pursue and achieve a higher quality of life that allows them to prosper in the community.
Responsibilities:
- Shares relevant personal recovery experience to provide hope and empowerment to encourage and facilitate change within clients using individual and group modalities including 12 step groups.
- Facilitates client support groups.
- Create recovery-oriented rapport with individuals with mental health or co-occurring disorders and connect them to wrap around services.
- Actively participates in community meetings to advocate for clients and to ensure the provision of integrated care.
- Coaches and provides supervision/direction with daily challenges such as personal hygiene, nutrition, housekeeping, personal budget and household management.
- Collaborates with care team on client engagement and treatment recommendations.
- Provides encouragement and recovery knowledge to family members/natural support.
- Carries a caseload based on program need.
Requirements:
- Must have a valid driver’s license.
- H.S. Diploma or GED required.
- Graduate of Recovery University or Certification by CCAR as a Recovery Coach required.
Preferred Qualifications:
- Beginner to intermediate experience with Microsoft Office products including Word and Outlook.
- Experience in an Electronic Medical Record (EMR) system.
- Experience working in Case Management or related field with a solid understanding of Mental Health conditions.
