Registered Nurse - Assertive Community Treatment

JOB SUMMARY

General Description: Provides health services, care management and care coordination to consumers through pre-evaluation, assessment, planning, education, intervention, and review. Participates as part of an inter-disciplinary team.

Responsibilities and Duties: Collaborates with primary health care providers, specialists, members of CMHCM health care team, including Medical Assistants, and consumers/families to ensure understanding of healthcare options and the delivery of quality, efficient, and cost-effective physical and behavioral health care services. Assesses, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the consumer's health status based on the principles of recovery and self-determination. Conducts nursing, OBRA and/or AIMS assessments. Develops person centered plans with consumers and provides consumer education. Obtains informed consents as needed. Coordinates and administers injectable medications and vaccinations. Assists and conducts medication reviews. Initiates first aid treatment when necessary. May participate on the ACT team. Provides nursing consultation. Contacts pharmacies and coordinates with primary health care physicians. Acts as resource person for health issues by interpreting lab work and other medical data and conducts necessary follow-up activity. Monitors consumer's for side effects, high risk medications, and consumer blood drug levels. May assist other Registered Nurses in other counties as needed. Serves as nursing advisor for community living facilities/AFC homes. Performs necessary record keeping according to agency standards. Performs related work as assigned.

Works individually, with CMH staff, and with the consumers identified in the targeted high-risk population. Monitors physical health care interventions and helps facilitate/consult on wellness plans for all consumers. Provides initial and follow-up health assessments, educational needs, and health care plans for identified consumers. Acts as a liaison between consumers and primary health care providers.

Responsibilities also include, but are not limited to:

  • Working with individual consumers on patient activation, health literacy, motivation, medication adherence, and healthy lifestyles. May include attendance at physical health appointments, role playing interactions with medical providers, and observing physician-consumer interactions.
  • Accessing health system physical health supports, including diabetic education, reviewing materials with consumers, and discussing barriers.
  • Coordinating with hospital discharge planner, health systems care manager, and Medicaid Health Plan case manager to ensure smooth transition of care; including medication reconciliation.
  • Population health research using various health information exchange platforms to identify individual consumer, as well as community needs.
  • Working closely with CMHCM Medical Director and Nurse Administrator to promote Integrated Care.


EMPLOYMENT STANDARDS

General Education and Job-Related Experience: Minimum of an Associate's Degree in Nursing and a Registered Nurse license in the state of Michigan is required. Experience working with individuals with mental illness or intellectual/developmental disabilities preferred.

Licenses/Certification: Michigan RN license and valid driver's license required. CPR certification preferred.

CONDITIONS OF EMPLOYMENT

Knowledge: Knowledge of specialized and general nursing principles and techniques; psychotropic medications, health screening and treatments; current literature in the field of nursing; and materials, supplies and equipment required.

Skills: Skill in use of telephone, writing, giving injections, checking blood pressure, Body Mass Index (BMI) and waist circumference measurement, handling nursing related instruments, use of electronic health record, automobile, crisis intervention, providing in-services, providing nursing assessments and establishing rapport with individuals with mental illness and/or intellectual/developmental disabilities.

Abilities: Ability to communicate with others, both verbally and in writing; ability to develop and maintain positive interpersonal relations, get along well with consumers, and show empathy; ability to demonstrate flexibility and adaptability, self confidence, and accept constructive criticism; ability to demonstrate organizational skills, maintain accurate records, and work as a team member. Ability to do AIMS screening, assess medication effects, work independently, and promote consumer self sufficiency; ability to provide necessary health care, assess health care needs of consumers and assess the effectiveness of nursing care; ability to educate and motivate consumers; and ability to carry out detailed instructions, maintain calm during stressful situations and maintain favorable public relations; ability to advocate for consumers independently and as part of a team; ability to learn and comply with federal, state, and agency policies and procedures.

Physical Demands: Employee is regularly required to stand, walk, sit and operate a computer, telephone, fax machine and copier and may require travel.

Work Environment: Work is performed in a typical office setting and other community settings and may involve dealing with people under very trying or difficult circumstances.

SCHEDULE

This is a full time position, 40 hours per week. The general schedule is Monday - Friday 8am to 5pm.

Salary Range: $48,880 - $66,518 depending on experience
