Essential Duties and Responsibilities:
To excel in this role, an individual must be able to perform the following tasks effectively:
- Deliver employment and/or housing support in alignment with individuals’ person-centered plans.
- Assist individuals in building support systems to achieve success in employment, housing, or both.
- Collaborate with other CME programs, including Plan to Work, State Opioid Response, Recovery Café, and Recovering Smiles.
- Act as a liaison between clients, community partners, housing providers, and employers.
- Provide high-quality services that meet the standards of quality improvement and utilization management systems.
- Document services accurately in client records while adhering to strict confidentiality standards.
- Conduct outreach and marketing to referral sources and community partners.
- Ensure timely and accurate documentation within the required billing system.
- Respect and uphold clients’ rights, cultural preferences, and individual needs.
- Represent the program by attending community meetings, outreach events, and trainings (in-person or virtual).
- Complete other responsibilities as assigned, including occasional evening and weekend hours.
Qualifications:
Education and Experience:
- AA in a related field OR two years of relevant experience; a combination of education and experience may be considered.
- Experience in providing Foundational Community Supports or other Recovery Services is a plus.
- Certified Peer Counselor or Recovery Coach certification preferred.
Certificates and Licenses:
- Current Washington State Driver’s License and access to a reliable vehicle with liability insurance.
- Ability to pass ongoing background checks.
- Must be a Washington Health Plan Finder Navigator or able to obtain certification within 90 days of hire.
Skills and Abilities:
- Language Skills: Fluency in additional languages is a plus. Strong communication skills and documentation knowledge required.
- Interpersonal Skills: Excellent communication skills and the ability to establish boundaries are essential.
- Computer Skills: Proficiency in Microsoft Office applications (Word, Excel, PowerPoint, Outlook), telehealth platforms (Zoom, Teams), and proprietary databases.
- Other Skills: Ability to conduct internet research and adapt to evolving technology needs.
Work Environment:
This role involves representing the program at community events, outreach activities, and meetings. Occasional evening and weekend hours may be required.
