LPN Intake Coordinator for Home Care

Community Nurse is looking for a full-time LPN Intake Coordinator to join our Intake Team, 35 hours per week with weekend and holiday rotation.

Requirements:

Position Education/License Requirements:

Currently licensed as a licensed practical nurse pursuant to Massachusetts General Laws CMR 2.01.

Skills/Experience Requirements:

• Minimum of 3 years of homecare experience
• Knowledge of home health regulatory system and reimbursement criteria.
• Computer experience
• Requires ability to work effectively with all departments, maintain confidentiality.
• Excellent communication skills

Description:

• Responsible for processing agency home health and hospice referrals in an accurate, timely manner, including accurately completing data input and client information as listed on referral
• Complete basic diagnosis coding by referral information.
• Serve as the liaison between agency and patient/caregivers.
• Track and follow up with all patients who are pending start of care.
• Understand and effectively communicate the services offered by the Agency
• Interact with referral sources and sales team regarding new referrals.

Job Type: Full-time

Pay: $29.00 - $30.00 per hour

Expected hours: 35 per week
