Registered Nurse

Registered Nurse - Weekends, 4pm to 9pm or 1pm to 9pm

Requirements:

• Licensed RN in Massachusetts
• Graduate of an approved school for professional nursing,
• A minimum of one year of recent experience in direct patient care nursing.
• Previous home care experience preferred.
• Experience with documentation in a patient Electronic Medical Record is a plus.

Responsibilities:

• Assumes on-call coverage during evening hours and responds to patient/family needs in accordance with Community Nurse policies and procedures.
• Able to assess and respond to the needs of patients and families in varied settings.
• Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
• Able to make meaningful observations and write comprehensive accurate reports.
• Communicates with attending physicians, other staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family.
• Maintain regular communication with the Clinical Manager, and/or other care team members concerning patient/family care needs.
• Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
• Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, or by providing direct care as appropriate.
• Provide appropriate support at time of death and period of bereavement.
• Participate in Community Nurse orientation and in-service training programs for professional staff.
• Participate in agency and community programs as requested to promote professional growth and understanding of home care.
• Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.
• Accept other assignments as appropriate.
