Nurse Practitioner

Community Nursing Services (CNS), a mission-driven, not-for-profit healthcare provider, is seeking a dedicated Nurse Practitioner to join our Home Health and Hospice team. This role focuses on delivering compassionate, patient-centered care to individuals in their homes. You'll have the opportunity to make a meaningful impact, ensuring comfort and quality of life for patients and families.

Key Responsibilities:

• Provide comprehensive assessments, develop care plans, and manage treatment for home health and hospice patients.
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure seamless care delivery.
• Prescribe medications, order diagnostic tests, and manage chronic and acute conditions.
• Educate patients and families on care plans, symptom management, and health maintenance.
• Support end-of-life care planning, including advance directives and pain management.
• Maintain accurate and timely documentation in accordance with CNS and regulatory standards.

Qualifications:

• Current Nurse Practitioner license in the state of Utah.
• Experience in Home Health, Hospice, or Palliative Care preferred.
• Strong clinical assessment and decision-making skills.
• Excellent communication, empathy, and patient advocacy abilities.
• Ability to work independently and adapt to diverse patient needs.
