Position Summary : The Early Intervention Social Worker, HealthySteps is an early child development expert who will join the Integrated Emotional Wellness Team, providing interventions, referrals, and follow-up for families with patients ages 0-3. This Specialist will work to educate patients' family members and colleagues about developmental resources and treatment options, building strong relationships with families, and facilitating referrals to appropriate services. This is a full-time position, reporting to our Director of Integrated Emotional Wellness and will be based out of our Family Health & Birth Center in NE, DC.
The salary for this position ranges between $75,000 - $85,000 annually and the offer amount is determined by the candidate's education, qualifications, and experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.
Highlighted Duties and Responsibilities :
Promotes processes and programs in fidelity to the HealthySteps (HS) model within the practice to families and providers.Conducts team-based well-child visits before, during, or following the primary care provider; visits focus on monitoring development, social-emotional functioning, relational health, screening for family / emotional needs, and / or providing anticipatory guidance.Conducts consultations on a short-term basis for needs regarding development and / or behavioral concerns such as sleep, positive parenting, picky eating, etc.Provides crisis intervention per the clinician’s licensing and COH guidelines, to include addressing suicidal / homicidal ideations.Ensures patient / family safety by following COH policies for contacting Child and Family Services Administration, Adult Protective Services, Mobile Crisis, and / or 911 when appropriate.Creates patient safety contracts as appropriate with the patient in crisis and stored within the patients’ chart by close of business.Maintains a daily schedule of at least 6 patients seen per day.Completes a treatment plan for all short-term patients / families by the 2nd scheduled session. Monitors family progress through the course of treatment, reevaluating and adapting the treatment plan at least monthly from initial treatment plan date.Participates in primary care team meetings and / or “huddles” regularly.Facilitates patient / family engagement and follow-up in care; sets actions per department guidelines in eCW to track follow up.Follows appropriate discharge and / or discontinuation of therapy procedures.Maintains safe, secure, and healthy work environment by following standards, procedures, and legal guidelines.Remains current with ongoing training requirements and CEUs as required for licensing.Provides clinical supervision to Master level interns and licensed graduate clinicians as appropriate.Creates and maintains community resource directory / database. Provides referrals and tracks follow-up, as appropriate, to help families make successful connections to key resources within the community.Facilitates and appropriately documents referrals for clinically indicated services outside the primary care clinic (e.g., domestic violence safe shelter, mental health specialty care, core service agency, and substance use treatment).Builds and maintains relationships with community providers to enhance knowledge about HealthySteps program and referral criteria; promote clinic-wide understanding of the HSS role and support full integration of HSS into the care team.Participates (when appropriate) in community-wide early childhood and / or mental health meetings.Works with the front office administrative staff and EW administrative staff as appropriate to ensure scheduling of the HS Specialist’s time in coordination with provider visits (i.e., well-child visits).Engages in Continuous Quality Improvement (CQI) initiatives to ensure the HealthySteps model is delivered with fidelity.Collaborates with HealthySteps implementation team to implement all eight Core Components including universal screenings, making positive parenting and early learning guidance information available, and adjusting workflows for optimal efficiency.Works closely with pediatric primary providers around care coordination, goal setting, coaching, and education about key aspects of a child’s development.Provides consultation and / or facilitates training to medical professionals and all practice staff re : early childhood development, early relational health / infant early childhood mental health, treatment plan changes, families / children who are not improving as expected and trauma-informed practice.Documents all materials given to the patient in the electronic record, including any document needing to be uploaded via patient docs within eCW per the established timeframe.Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.Maintains extensive databases required to meet HealthySteps fidelity metrics including both internal and external referrals.Collaborates with HealthySteps Implementation Team to complete required annual site reporting to the HealthySteps National Office at ZERO TO THREE, which may include analysis of both external database files and EHR data reports.Tracks caseload to ensure capacity to deliver the HS services within the risk stratified service delivery model. This includes ensuring exit criteria are met and that families and children are elevated to higher levels of care or referred to community providers if needed.Participates in HealthySteps training (Virtual HealthySteps Institute), annual model refresher course, and other model trainings as needed.Completes HealthySteps Competency Based Professional Development Assessment and develops and implements a professional development plan that is updated annually.Minimum Qualifications :
LGSW / LICSW, LGPC / LPC, required or comparable license. Any providers who are licensed in Maryland who provide a copy of their license and MD CDS to Talent Management are also eligible for MD licensure reimbursement.A minimum of two years of work experience in a clinical environment preferred.Strong organizational skills.Demonstrated ability to collaborate effectively in a team setting.Experience and knowledge about early childhood growth and development, parent-child relational health, infant and early childhood mental health, and family systemsExperience with assessment and treatment planning for common mental health and / or substance use disorder requiredFamiliarity with brief, structured intervention techniques required.Experience working with underserved population preferred.Ability to work flexible hours, including evenings and weekends, required.May need to travel to other sites.Proof of required vaccinations is required.
