Position Summary:
We are seeking a passionate individual who is eager to serve underserved individuals in DC and values the impact that having hope can offer. We follow a Housing First model and philosophy, and work to keep families – all along the continuum of homelessness – safe with a roof over their heads. As a Housing Stability Case Manager, you will give families the support they need at the most pivotal times, building on families’ strengths, honoring their choices, and taking a whole-family, multi-generational approach. You will provide housing stability and income-focused case management services to the families assigned to your case load. You will have a direct impact in people’s lives.

The pay rate for this position ranges between $25.96 - $26.92 per hour and the offer amount is determined by the candidate's education, qualifications, and experience . really provides their own estimated salary calculator and is not affiliated with COH's range.

Highlighted Duties and Responsibilities:

  • Works with adults and children to develop case management plans that are updated at least monthly and signed by clients and case manager, which incorporate goals for both adults and children related to core goal areas: finding stable housing; increasing income; and family stabilization and support;
  • Meets weekly and as needed with families on caseload to develop case plans, review progress, and to follow-up on tasks and services;
  • In collaboration with families and Housing Specialist, assists with housing search and placement process, sometimes accompanying families to view units, advocating for families with landlords, and assisting families with applications and other paperwork;
  • Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, etc., and makes referrals as appropriate, including to COH Therapist and other community resources, as appropriate;
  • Completes required assessments with families;
  • Meets regularly with Youth Specialists to ensure children’s needs are part of the parent’s case management plan;
  • Obtains signed release of information forms and communicates regularly with staff at treatment programs, substance abuse programs, and vocational programs.
Requirements:

Minimum Qualifications:

  • Bachelor’s degree in social work or related field required or 4 years social service experience.
  • Minimum one year work experience in social service or related field preferred.
  • Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly preferred.
  • Strong interpersonal, conflict-resolution, and organizational skills required.
  • Ability to work independently and as a team required.
  • A valid driver’s license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.

  • In relation to remote work versus on-site expectations, this position is classified as the following:
  • Hybrid: Remote work and on-site work requirements are generally split roughly 50/50. Please note that remote work designations are subject to change or fluctuate at any point in time and the supervisor may require in person learning for a specific amount of time after hire.

Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved and people experiencing homelessness in Washington, DC. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC. In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients. Community of Hope provides community walk-in COVID testing and COVID vaccines. Community of Hope is also one of the largest providers in DC of housing and support services for families and individuals experiencing homelessness. Through providing these programs, we live out our mission to improve health and end family and individual homelessness to make Washington, DC, more equitable.
