Position Summary:
The Housing Stability Case Manager position is accountable directly to either the Team Coordinator or Program Director. This is a full-time position based out of The Triumph in SE DC.


Salary – The salary range for this position is $25.96 – 26.92 per hour
and the offer amount is determined by the candidate's education, qualifications, and experience. really provides its own estimated salary calculator and is not affiliated with COH’s range.


Highlighted Duties and Responsibilities:

  • Welcomes families to the program, which includes reviewing and explaining program rules and client rights and responsibilities and providing each family with a copy of the Program Rules.
  • Works with families using a two-generation (whole family) approach to develop individual goal plans that are measurable and detailed with action steps that will help each family achieve their goals. Updates goal plans according to program contract guidelines and as needed, signed by client and case manager.
  • Meets with families on caseload to support implementation of their goal plans and follow-up in accordance with program contract guidelines and family needs.
  • Assists families with housing search process in collaboration with Housing Specialist. As needed, accompanies families to view units, advocates for families with landlords, and assists families with applications and other paperwork. If COH is providing financial assistance for application fees, accompanies families to render payment.
  • Maintains regular communication in accordance with program contract guidelines with all families on caseload and documents progressive engagement efforts with difficult to engage families.
  • Works consistently within a housing-first framework (i.e., providing clients with immediate access to available independent permanent housing and supportive services without prerequisites for sobriety or participation in psychiatric treatment), ensuring that services offered are strengths-based and client-centered.
  • Meet face to face with families on caseload to pursue goal-related tasks, review progress, and address barriers to accessing services;
  • Enters data into all appropriate databases, according to the schedule required by funding sources and Community of Hope.
Requirements:
  • Associate's degree or two years of college.
  • In lieu of the education requirement, a minimum two years of experience with homeless services programs or provision of social services is required.
  • Bachelor’s degree preferred;
  • BSW and MSW degree holders must have a LSWA/LGSW or appropriate social work license prior to starting.
  • Minimum of two years of experience working in housing/homelessness or social services field;
  • Strong knowledge of community resources or ability to become knowledgeable required;
  • Strong interpersonal and organization skills required;
  • Strong communication skills including verbal and writing;
  • Proven ability to submit timely, detailed and accurate case notes.
  • Bilingual: Fluent in English and Spanish Preferred.
  • In relation to remote work versus on-site expectations, this position is classified as the following:

Hybrid: Remote work and on-site work requirements are generally split roughly 50/50.

About Us:

Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For 45 years, we have provided healthcare, housing, and community support services to make Washington, DC more equitable. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC. In 2024, Community of Hope provided about 50,000 medical visits, 6,300 dental visits, and 17,000 behavioral health visits for about 16,000 patients. Community of Hope is also one of the largest providers of homelessness prevention and housing services for 1,600 households—1,384 families and 220 individuals—experiencing homelessness in DC. Community support through Family Success Center, our WIC nutrition centers, and perinatal care coordination for pregnant people experiencing homelessness reach hundreds more families.
