Position Summary:
We are seeking a passionate individual who is eager to serve underserved individuals in DC and values the impact that having hope can offer. We follow a Housing First model and philosophy, and work to keep families – all along the continuum of homelessness – safe with a roof over their heads. As a Youth Focused Case Manager you will coordinate efforts of Case Managers and Parent(s) to stabilize youths’ behavior and provide resources and supports for youth to succeed in educational and recreational settings. The Youth Focused Case Manager you will give families the support they need at the most pivotal times, building on families’ strengths, honoring their choices, and taking a whole-family, multi-generational approach.

Highlighted Duties and Responsibilities:
- Provides short-term case management services for up to 15 highly vulnerable youth of families currently enrolled in Permanent Supportive Housing (PSH) Program.
- Works with Case Manager and Parent(s) to perform intake evaluations of youth upon referral and ensures appropriate services for children.
- Works with families using a two-generation (whole family) approach to develop case management plans that set positive short-term goals with action steps that help each family achieve their goals. Update’s goal plans according to program contract guidelines and as needed, signed by Parent(s) and/or child and youth-focused case manager.
- Meets with families on caseload to support implementation of their goal plans and follow-up in accordance with program contract guidelines and family needs.
- Ensures that children are enrolled or reinstated into appropriate school settings or job training programs and are compliant with court orders and treatment plans.
- Meets regularly with Case Managers and/or Team Coordinators to discuss client case plans and progress including relevant issues.
- Maintains appropriate files, including updated case notes, incident reports, rule violations, etc., according to the specifications in the PSH Case Management protocol.
- Works with Case Managers on a weekly basis to ensure that children’s needs are reflected in case management plans and being addressed by Parent(s).
- Enters case notes and data each week into HMIS / OCTO and provides information at least monthly for funder reports and any other reports, such as case reviews.
- Serves as liaison between parents and school systems in the implementation and facilitation of the IEP process and all other school related issues.
- Creates relationships with attendance officers, school and employment counselors and conducts regular school, home, and community visits.
- Creates partnerships between neighborhood/city agencies that have children/youth programs and serves as liaison between them and COH. Provides those resources to Home Now Program.
- Attends clinical and Housing Department staff meetings and other organizational meetings as appropriate.
- Meets quarterly with case managers to review attendance records. Provides resources/support to case managers around attendance issues as a result of these meetings.
- Completes and distributes monthly youth specialist newsletter.
- Conducts ASQs as needed and upon request from clients and case managers.
- Attends as-needed meetings of youth-focused staff across Housing programs to foster cooperation and shared resources and identify opportunities for collaboration .
- Makes referrals for health services to Community of Hope’s clinic as needed.
- Works with development office on fund raising needs and responds to requests for information on a timely basis.
- Meets regularly with social workers or case managers in other programs to share resources and information.
- Assists with execution of annual Back to School event, as well as all other youth related events.
Minimum Qualifications:
- Bachelor’s degree or minimum four years’ experience working with high-risk youth ages 3-24 in an urban setting required.
- Strong cross-cultural and interpersonal skills.
- Competent computer and internet skills.
- Ability to work well with parents as well as youth.
- Strong initiative.
- Strong organizational skills.
- Ability to work independently.
- A valid driver’s license and reliable transportation required, as well as proof of auto insurance.

Hybrid: Remote work and on-site work requirements are generally split roughly 50/50.
