Job Title: Licensed Practical Nurse
Location: River Edge, NJ
We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Community Options, Inc. in River Edge, NJ. As a LPN, you will deliver care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
Responsibilities:
- Provide treatment and healthcare procedures ordered by a physician and required by professional practice or law
- Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
- Advise supervisor of all concerns and issues related to nursing services
- Train employees on medical needs of assigned individuals
- Monitor health and medical conditions of individuals and refer for medical services as needed
- Build support networks for individuals by developing and maintaining relationships with community members, businesses, and families
- Cooperate with license and department staff in inspections, inquiries, or investigations
Requirements:
- Bachelor's degree or equivalent experience in the nursing field
- Minimum of one year of experience providing nursing services to individuals with disabilities
- Valid nursing license
