Job Summary
We are seeking highly motivated and compassionate individuals to join our team as Full-Time Community Integration Counselors in Drums, PA. As a Community Integration Counselor, you will play a vital role in assisting individuals with learning physical, intellectual, emotional, and social life skills.
Responsibilities
- Support and encourage individuals to increase their ability to be as independent as possible by developing personalized plans that cater to their unique needs and goals.
- Maintain regular contact with waiver participants in various settings, including their residences, the community, or offices, to provide ongoing support and guidance.
- Collaborate closely with Case Managers to create and facilitate programs that address the social, emotional, physical, and personal needs of individuals.
- Contribute to community integration efforts by providing resources, information, and advocacy to help individuals access community-based services and opportunities.
- Document individual progress toward acquiring community integration skills, ensuring accurate and timely reporting.
- Establish strong relationships between individuals and their communities by building a network of support, including family members, friends, and community organizations.
- Ensure medical consultation forms are completed by physicians for each medical appointment attended and administer medications according to established procedures.
- Coordinate leisure and volunteer activities that promote socialization, skill-building, and overall well-being.
- Monitor health and medical needs, promptly reporting any concerns or issues to ensure prompt attention and support.
- Transport individuals to medical appointments and community activities, utilizing company vehicles or personal vehicles as needed.
Requirements
- A High School Diploma or equivalent is required; a Bachelor's Degree is preferred.
- A valid driver's license with a satisfactory driving record is necessary.
- A minimum of 2 years of experience supporting individuals with intellectual and developmental disabilities is required.
- The ability to establish trust, empathy, and effective communication with individuals receiving supports is essential.
- Excellent problem-solving skills, judgment, and crisis management abilities are necessary to handle challenging situations effectively.
