We are Now Hiring a Qualified Intellectual Disability Professional in Brooklyn, NY to provide person-centered care tailored to meet the needs of individuals with intellectual and developmental disabilities. The Qualified Intellectual Disability Professional assists the Executive Director in developing and maintaining individual service plans that meet the needs of people with disabilities and state and area
program guidelines.
Starting pay is $23.00 / hour
Responsibilities
- Develop individual service plans (ISP) / Person Centered Plans (PCP)
- Coordinate admission and discharge activities of participants
- Act as a liaison for area programs with clinical issues
- Provide support and supervision to Sr. Community Support Staff regarding compliance and clinical guidelines
- Prepare monthly goal progress notes and billing justification forms
- Maintain and analyze all incident / accident reports
- Facilitate selection of clinical consultants, and monitor their activity
- Coordinate behavioral intervention plans, to include routine and emergency services
- Monitor medication procedures, administration, and documentation
- Evaluate participant satisfaction
- Monitor staff compliance with plan implementation and licensing compliance
- Evaluate staff performance
- Employee must cooperate with the licensee and department staff in any inspection, inquiry or investigation
- Additional tasks and responsibilities may be assigned
