About the Role
The Community Integration Counselor will be responsible for meeting with waiver participants regularly to develop and facilitate programs that meet their social, emotional, physical, and personal needs. You will work closely with Case Managers to provide ongoing support for community integration efforts and document individual progress toward acquiring community integration skills.
You will also assist in building a support network between the individual and the community, ensuring medical consultation forms are completed by physicians for each medical appointment, administering medications, and completing necessary documentation.
