Case Management Support
Community PACE at Home in Newaygo is seeking a Case Management Support person. We need an individual that is passionate about the wellbeing of our elderly population. The person in this position will be responsible for the direct social work services to the participants of the Community PACE at Home program. This position is in-person, full-time, Monday through Friday. We’re looking for a compassionate, caring, and patient individual, that loves our elderly community to join our family-oriented team.
Community PACE at Home in Newaygo is now under a new management team. The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. Community PACE is proud to deliver all needed medical and supportive services. Our PACE program can provide the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their home for as long as possible.
By joining our team, you will be responsible for the direct social work case management services to the participants of Community PACE at Home. You would work under the direct and indirect supervision of the Clinical Director/MSW. You would provide social services support such as participant screening, case management, counseling, and referral.
Requirements:
· Knowledge, understanding, and experience working with Medicare and Medicaid.
· Bachelor's Degree in Social Work or related field is preferred.
· At least one year of social work experience and one year of experience with a frail elderly population.
· Must possess a valid Michigan Driver License, dependable transportation, CPR and First Aid certification.
· Must thrive in a team environment, be self-motivated, well organized, and can multi-task effectively.
Key Responsibilities:
· Monitor Medicaid and Medicare for all enrolled participants.
· Assist in completing Medicaid re-determinations and keeping a line of communication with PACE case worker through DHS.
· Update and maintain charting records for case management and family communication.
· Schedule and conduct family meetings when necessary.
· Implement Behavior Contract with participants in frequency determined by IDT.
· Aiding participants in completing DPOA paperwork and addressing family concerns when enactment is requested.
