Master of Social Work
Community PACE at Home in Newaygo is seeking a Social Worker / Case Manager to serve our elderly community. We need a person that will be responsible for the direct social work case management services to the participants of the Community PACE at Home program. This position is in-person, full-time, Monday through Friday. We are looking for a compassionate, caring, and patient individual to join our team.
The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. Community PACE is proud to deliver all needed medical and supportive services. Our PACE program can provide the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their home for as long as possible.
By joining our team, you will be responsible for the direct social work case management services to the patients of Community PACE at Home. You would work under the direct and indirect supervision of the Executive Director. You would provide social services support such as participant screening, case management, counseling, and referral.
Requirements:
· Master's Degree from an accredited School of Social Work.
· 2 years’ experience in a health-related area. Minimum of 1 year experience working with the frail elderly population. Experience working with Medicaid services and populations strongly preferred.
· Must possessLMSW, valid MI Driver License and vehicle, CPR and First Aid certification.
· Individual therapy skills for geriatric participants, analytical thinking, and problem-solving abilities. Includes home visits to private homes. Only act within the scope of his or her authority to practice.
· Immunizations: You must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Key Responsibilities:
· Maintains current written case management records.
· Provides individual and family counseling, develops, and leads group counseling and activities.
· Refers patients and families to appropriate community agencies or facilities, acts as liaison with such organizations and advocates for participants.
· Consults with and advises staff members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of social services in the community.
· Participates in Program and Policy development of Social Work component.
· Facilitates Medicaid application and re determination process with Michigan Department of Human Services.
· Documents patient changes appropriately in the medical record.
· Attends staff meetings and takes part in patient care planning.

Compensation is based on experience and is negotiable.
