The Role:
As a Case Manager, you will be responsible for coordinating and facilitating service planning for individuals with Developmental Disabilities, while working collaboratively with other community resources to ensure high-quality service provision.
What We're Looking For:
•An associate degree or equivalent; a bachelor's degree is preferred
•Two-to-three years of relevant experience; a background in social work, counseling, or a related field is an asset
•Excellent communication skills, both written and verbal
•The ability to utilize technology provided to perform job requirements
•A valid driver's license and proof of vehicle liability coverage
•Strong multitasking abilities and attention to detail
