The Role:
As a Licensed Practical Nurse at Community Partners, you will be the primary nurse for an assigned caseload of individuals. You will work collaboratively to ensure the health and safety of those on your caseload. You’ll train staff and provide client education to ensure that all healthcare needs are being addressed. You’ll also act as a liaison between the agency and medical providers, communicating, scheduling appointments, and advocating as needed.

You are a health advocate and believe that everyone deserves to have all their healthcare needs addressed. Your bright personality helps to easily build relationships with others and being a team player is your game!
What we’re looking for:
• Currently have NH LPN license in good standing
• 2 years of nursing experience
• Excellent oral and written communication skills
• Ability to utilize technology provided to perform requirements of the job
• Hold a valid driver’s license and proof of vehicle liability coverage.
