POSITION SUMMARY
To provide adoption case management and support to children available for adoption, including recruitment and family matching efforts to achieve permanency, to coordinate and ensure necessary services are provided to address well-being and permanency needs, responsible for Case Management of adoption case for children in unidentified placements, including safety assessments and ensuring well-being needs are met.

POSITION SUMMARY
To provide adoption case management and support to children available for adoption, including recruitment and family matching efforts to achieve permanency, to coordinate and ensure necessary services are provided to address well-being and permanency needs, responsible for Case Management of adoption case for children in unidentified placements, including safety assessments and ensuring well-being needs are met.
PRINCIPAL DUTIES AND RESPONSIBILITIES
- Ensure completion of Child Studies for permanently committed children with the goal of Adoption.
- Utilizes effective and accountable recruitment activities for children who are available for adoption who do not have an identified adoptive family by utilizing available tools/resources for Child-Focused recruitment.
- Develops and maintains case records and service documentation according to contract and Community Partnership for Children standards (JRSRRS, case plans, chronological notes, etc.)
- Consistently documents case activities within 24 hours of event.
- Arranges for emergency placement, emergency medical treatment, and emergency services for children in care.
- Completes and submits court documentation within in required time frames.
- Obtain all relevant documentation for the purpose of adoption disclosure.
- Maintains documentation on prospective adoptive families and maintain adoption files for children.
- Prepares for, attends, and participates in all court activities, including testifying in court as necessary.
- Monitor quality of care by conducting face-to-face home/residential visits of the child every 25 days.
- Assists in developing child-specific recruitment plans for children with an emphasis on children who have been permanently committed longer than 6 months.
- Conducts child-specific Adoption home studies for prospective adoptive families.
- Provides and/or refers children to services, understand and use information from service providers, psychological reports, and psychosocial evaluations.
- Maintains regular contact with service providers and documents services progress in FSFN.
- Provide and/or arrange for transportation of clients to appointments, community agencies, etc., as needed.
- Ensure children’s safety and well-being needs, including medical, dental, mental/behavioral health, are addressed, and maintained while in out of home care.
- Organizes, prioritizes, and completes assignments by established deadlines.
- Assists in the preparation and presentation of unidentified children by participating in bi-monthly Match staffings.
- Arranges for, attends, and participates in individual case staffings, as necessary.
- Provides and coordinates support to foster homes and relative/non-relative placements.
- Participate in the quality improvement process.
- Serve as on-call case manager after hours as assigned and determine and respond to immediate risk and needs of child.

EXPERIENCE/PERFORMANCE REQUIREMENTS (Knowledge, skills, and abilities):
- One or more years previous case management experience working with trauma-impacted children
- Ability to attain Child Welfare Credentials from the Florida Certification Board
- Adoptions, recruitments and/or marketing experience, preferred.
- Strong organizational, prioritization and problem-solving skills.
- Ability to establish and maintain positive working relationships with others.
- Ability to maintain confidentiality of sensitive data.
- Effective listening, verbal, and written communication skills.
- Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
- Must work well independently, as well as can collaborate with others.
EDUCATION REQUIREMENTS:
Bachelor’s Degree from an accredited college or university in a human service-related field. Or Associate’s Degree and 2 years related experience with at risk children and families.
LICENSES AND CERTIFICATIONS
- Possess a current Background Clearance Screening Letter as required by DCF; and successfully complete the background screening requirements for Community Partnership for Children.
- If local travel is required, a Valid Florida driver’s license and documentation of current automobile insurance, with $100,000/$300,000 bodily injury liability limit, is required
PHYSICAL DEMANDS AND WORK ENVIRONMENT
Employee will work in an office/clerical environment. The employee will work predominantly seated with recurrent need to walk, stand, and bend from the waist. Occasional light lifting, stooping, and climbing may be required. Occasional local travel.
