What are the responsibilities and job description for the Nurse Case Manager (Remote) position at Community Physicians?
Qualifications
- Must be an Illinois resident
- Licensed Practical Nurse currently licensed in the State of Illinois
- One (1) year of nursing experience required.
- Experience in acute, post-acute care, and/or long-term care preferred.
Responsibilities
- Develop care plans based on mutual goals with patient, family, physician, and documentation in medication record. Promote patient compliance with care plan and revise the care plan as needed.
- Coordinate with the interdisciplinary team to facilitate appropriate care for the patient's needs.
- Collaborate with patients, their families/caregivers, healthcare professionals and community resources to develop a comprehensive plan of care that promotes health and meets the patient’s care goals.
- Provide education to improve patient and family health literacy on patient’s chronic conditions.
- Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support.
- Provide timely communication and follow up with patients, families, and healthcare providers regarding changes in patient condition.
- Serve as a point of contact, advocate, and informational resources for patient, family, care team and community resources as needed.
- Assess the patient’s and family’s unmet health and social needs.
- Help patient and family with medication management and adherence to follow-up appointments.

Pay: $30.00 per hour
