The position will include a focus on nursing facility visits with a mix of outpatient and inpatient hospital consults.

Dekalb and Kane Counties!
The provider must work efficiently to provide quality psychiatric care in multiple settings including outpatient clinic, skilled nursing/long term care facilities, and hospitals.
· Focus on proactive, evidence-based treatment strategies that improve the health and well-being of individuals.
· Communicate effectively with patients, physicians, nurses, therapists, social workers, and other staff as necessary to assure proper diagnosis and treatment.
· Obtain, compile and record patient medical data, including health history, progress notes and results of physical examination.
· Ensure efficient and thorough documentation of visits as required per Medicare guidelines.
· Review and follow up with labs and diagnostic orders and determine appropriate treatment regimen.
· You will have the support of team staff to assist with administrative and billing processes.
· Structured physician support, training and oversight
Requirements: Must possess valid NP or PA license or be license eligible in State of Illinois.
- SNF/LTC experience preferred. If not, can train!
