JOB DESCRIPTION

 

   POSITION TITLE:  Community Response Clinician

   DEPARTMENT:   Clinical Services

    FLSA STATUS:   Exempt

    REPORTS TO:  Lead Clinician III



GENERAL DESCRIPTION OF DUTIES

 

The Community Response Clinician Level 2 is responsible for responding to crisis calls. This individual provides specialized crisis response services and/or one on one acute counseling services to participants that call for Police assistance due to experiencing a crisis.  Strong background on substance use, trauma, crisis response, and/or motivational interviewing skills is required.  

 

SPECIFIC DUTIES AND RESPONSIBILITIES

 

ESSENTIAL JOB FUNCTIONS

  • Provide brief immediate crisis intervention.
  • Provide acute counseling services until participants engages in treatment program or no longer needs services.  
  • Provide assistance and education in activities of daily living, support for employment/training search and job coaching.
  • Facilitates strong innovated, culturally diverse thinking and aptitudes to adjust to the population and interests of the group.
  • Educates participants on risk factors of behaviors to facilitate change in thinking.
  • Assesses and provides diagnostic evaluations to patient and referral sources.
  • Monitors participants/clients in the facility.
  • Ensures adherence to infection control practices.
  • Participate in regulatory Audits and or funders.  
  • Manages a fluctuating caseload of active / discharged participants on a daily and timely basis.
  • Ensure referrals and support services are made to outside programs for case management and medical intervention if applicable.
  • Orients new participants  to treatment program(s).
  • Refers participants to the agreed upon treatment program and participates in the initial treatment planning with the identified treatment provider.  
  • Provide participants with information about other Agency programs.
  • Develop, implement, and evaluate a comprehensive crisis plan with each participant maintaining regular contact until participants engages into treatment/community resource or declines need for further assistance.
  • Re-assess participants to formulate treatment/community resource needs.
  • Provide weekly office visits to program participants as applicable.
  • Maintain follow up contact and documentation requirements for every program participant
  • Maintain participant/client confidentiality.
  • Provide on-going liaison, referral and advocacy for Participants.
  • As part of professional development attend all trainings and meetings as assigned.
  • Participate with community providers as it relates to programs contents area.
  • Completes referral progress reports and/or closing/discharge summaries.
  • Maintains and disseminates listings of local AA, NA and CA meetings as needed.

 

ADDITIONAL JOB FUNCTIONS

 

  • Will require use of crisis intervention and motivational interviewing.
  • Attend a minimum of credentialing hours of substance use related training per year.
  • Maintain compliance with Complete yearly mandatory training including confidentiality and HIPAA compliance.
  • Conducts presentations for/at assigned prison regarding program resources.
  • Must be available to work evenings, weekends, and other shifts as assigned.  
  • Flexibility to work in various settings such as persons place of residency
  • Perform other duties as assigned.

 

 

MINIMUM TRAINING EXPERIENCE AND MEDICAL CERTIFICATIONS

 

Education:   Bachelors and/or Master’s degree in social work or related field from an accredited institution.

 

Minimum Years of Experience: Over three (3) years of experience working with marginalized populations.

 

Demonstrate skills: Proficient in databases, MS Office, written reports and maintenance of case notes. Strong organization, motivational, time management and interpersonal skills. Specialized training in crisis intervention,  substance use and/or mental illness, domestic violence and/or any other applicable training related to field of work preferred. Must have the ability to facilitate groups and a strong background in counseling. Experience working with participants from diverse ethnic and economic backgrounds with the ability to interact and communicate effectively.

 

Knowledge: Knowledge of Human Services, Criminal Justice, Court systems, substance use and target population(s) a must. Thorough knowledge of co-occurring disorders and of social services in Hartford with the demonstrated ability to work in collaboration with other service providers required.

Active Driver License in good standing required upon hire and throughout employment

Physical Ability: Tasks involve sedentary to light work, involving some reaching, handling, fingering and/or feeling of objects and materials.

 

Sensory Requirements: Some tasks require visual perception and discrimination. Some tasks require oral communications ability.

 

Environmental Factors: Tasks are regularly performed with exposure to adverse environmental conditions, such as dirt, dust, pollen, odors, wetness, humidity, rain, fumes, temperature and noise extremes, machinery, vibrations, electric currents, traffic hazards
