Job Summary
We are seeking a highly motivated and experienced Program Lead to join our team. The successful candidate will be responsible for creating a nurturing environment, developing strategies to support clients, and facilitating their independent performance in daily living areas. This is a dynamic role that requires strong organizational skills, effective communication, and a passion for improving the lives of others.
Key Responsibilities
- Create a positive learning environment and develop strategies to support clients' needs.
- Facilitate independent performance in daily living areas, including social, emotional, and physical situations.
- Implement individual plans of care, documenting progress and barriers to success.
- Maintain awareness of individual treatment plans, including leisure activities, community experiences, and safety skills.
- Monitor medical, physical, and behavioral changes, reporting concerns to supervisors or RNs as necessary.
- Administer medication, following agency policy and regulatory standards.
- Assist with meal planning, preparation, and cleanup, ensuring dietary plans are followed.
- Maintain the appearance, cleanliness, and upkeep of residences and vehicles.
- Conduct self in a professional manner, interacting effectively with participants, supervisors, and community members.
- Participate in training opportunities, maintaining required certifications and knowledge.
Requirements
- A high school diploma or GED and at least two years of experience in human services.
- Bachelor's degree in a related field, plus experience, is preferred.
- Valid CT driver's license and good driving record.
- Reliable transportation and ability to transport clients, if needed.
- Ability to perform active work, both inside and outside of the program.
