About the Role
The Shared Living Coordinator will play a vital role in supporting individuals with developmental disabilities in their homes, ensuring they receive the best possible care and services.
Key Responsibilities:
- Case management: Complete ISP assessments, coordinate medical care, audit finances monthly, and communicate with team members.
- Placement monitoring: Visit and monitor shared living placements once a month, submitting reports to icentrix.
- Staff supervision: Train providers and job coaches, supervise support staff, and provide performance reviews.
- Budgeting: Manage client funds, mileage, respite, and support staff hours.
- On-call support: Provide on-call support and be primary on-call at scheduled times.
- Data submission: Submit behavioral data monthly.
- File maintenance: Keep confidential files up-to-date.
Requirements:
- Proficiency in Microsoft Word.
- Effective communication and interpersonal skills.
- Strong telephone and problem-solving skills.
- Ability to maintain confidentiality and professionalism.
Preferred Experience:
