Job Summary
The Shared Living Coordinator will provide direct oversight and monitoring of assigned shared living placements, supervising support staff to ensure the highest quality care for individuals with developmental disabilities.
Key Responsibilities:
- Assume case management responsibilities, including completing ISP assessments, coordinating medical care, auditing finances monthly, and communicating with other team members.
- Visit and monitor shared living placements minimally once per month, submitting monthly monitoring reports to icentrix by the end of each month.
- Ensure providers and job coaches are fully trained in CPR, First Aid, Med in-service, Human Rights, fire safety, OSHA, behavior plan, on call, etc.
- Supervise support staff, formulate job descriptions for each support staff and client (if working), and provide provider oversight, including hiring, training, and performance reviews, in conjunction with the Director of Shared Living.
- Maintain budgets, including mileage, respite, and support staff hours, as well as managing client funds.
- Provide on-call support and assume the role as primary on-call at scheduled times.
- Submit behavioral data monthly.
- Maintain confidential files in accordance with the regulations of the Department of Development Services.
