The designated coordinator will play a vital role in coordinating and overseeing program activities and individual service delivery for individuals with disabilities or persons aged 65 and older. This position requires a comprehensive skill set encompassing program management, supervision, and evaluation.
Responsibilities:
1. Program coordination and Management:
- Coordinate service delivery for individuals served by the program.
- Oversee program activities to ensure compliance with relevant statutes.
- Evaluate program quality and implement improvements as needed.
- Maintain a current understanding of licensing requirements and ensure compliance throughout the program.
2. Supervision and Oversight:
- Supervise and evaluate individual service delivery.
- Implement corrective action following incident and emergency reports review.
- Ensure staff competency requirements are met through education, training, and relevant work experience.
3. Individual Service Delivery coordination:
- Oversee the company's responsibilities in support plans and addenda.
- Facilitate the accomplishment of outcomes according to statutory requirements.
- Instruct and assist direct support staff in implementing support plans.
- Evaluate the effectiveness of service delivery, methodologies, and progress on individual outcomes.
4. Compliance and Corrective Action:
- Ensure the fulfillment of designated coordinator duties.
- Conduct internal reviews of incident reports of alleged or suspected maltreatment.
- Take corrective action as ordered by the commissioner and ensure compliance with license terms.
5. Satisfaction and Improvement:
- Evaluate satisfaction of persons served, legal responsibilities, and case managers with service delivery.
- Develop, document, and implement ongoing program improvements based on evaluations.
Qualifications:
- Bachelor's degree in a field related to human services, and one year of full time work experience providing direct care services to persons with disabilities or persons aged 65 and older, or equivalent education and experience.
- Minimum of three years of supervisory level experience in a program providing direct support services.
- Competence in program management, oversight, staff supervision, and coordination of individual service delivery.
