Ryan White Medical Case Mnager/ plays a critical role in supporting healthcare services, patient education, and medical case management for individuals living with or at risk of HIV/AIDS. This position is responsible for facilitating patient-centered care, managing disease, and ensuring compliance with federal program requirements under the Ryan White and EHE initiatives.
· Qualifications & Requirements:
Education & Experience:
- Required: Licensed Practical Nurse (LPN) with an active, unrestricted license in the state of Florida
- Preferred: Associate Degree in Nursing (ADN) or equivalent accredited nursing program.
- Minimum care, infectious disease, community health, or case management.
- Experience working with diverse populations, including LGBTQ communities, individuals experiencing homelessness, and patients with mental health or substance use disorders.
Job Type: Full-time
Pay: $22.00 - $25.00 per hour
