LOCATION : Savannah, Georgia (Chatham County Area)
The Community Service Board of Middle Georgia is seeking a qualified individual to serve as an Intake Worker in our Adult Outpatient Services department. Intake Workers will provide linkage with external and internal programs regarding referrals. Complete follow-up on no show appointments. Complete Biopsychosocial and / or Addiction Survey Index Assessments on assigned cases. Obtain appropriate release of information and clinically significant information from various referral sources. Release clinically relevant information to various referral sources according to present administration policies. As well as other responsibilities.
Responsibilities of the Intake Worker :
- providing crisis intervention, screening, and counseling for consumers who contact the CSBMG for services and / or emergency screening consultations at the request of local hospital and Law Enforcement Agencies. Use clinical judgement to decide urgency of appointment.
- Provide quality linkage to and from various referral sources and communicate within programs to ensure continuity of care between Access and other CSBMG programs.
- Assist in the development of this initial treatment plan and make sure all duties are performed according to current quality assurance CARF standards and other applicable policies and procedures.
- Maintain up-to-date medical records according to current quality assurance standards. Assume clinical management of the chart until it is referred to another LOC or terminated.
- Check the data base / forms to make sure all accurate information is in the chart prior to leaving the Access Unit.
- Participates in psychiatric evaluation, assessment, and triage for individuals seen in the outpatient clinic.
- Documents clinical, collateral and psychosocial information to be used in disposition and diagnosis.
- Provides interventions such as crisis intervention, safety planning, or to facilitate referrals for ongoing treatment
- Provides an approach with individuals and families that are culturally competent.
- Interact effectively with individuals of different cultures including socioeconomic status and more specifically related to the demographics of their local community.
- Develops and maintains collaborative relationships with a variety of community and hospital resources in the region who are involved in the care of individuals with behavioral health issues.
- Ensures that proper treatment and referral protocols are followed with specialty clinics, and referrals.
- Documents intervention in individuals’ chart in compliance with division, department and agency policy.
