Job Duties / Responsibilities :
- Transport consumers as needed and complete daily documentation such as observation sheet, meal sheet, etc.
- Conduct 30 minute security checks and physician ordered special precaution checks.
- Conduct groups as scheduled and chart on groups in Netsmart and in the group book, as needed.
- Obtain vital signs at 0600, 1000, 1400, 1800, & 2200. Chart vital signs in Netsmart. Report abnormal vital signs to nursing staff.
- Take consumers on smoke breaks. See smoking policy for times.
- Assist with new admissions i.e. take vital signs, clothing inventory, complete admission paperwork from start to finish.
- Assist with discharges i.e. inventory belongings prior to discharge, ensure all personal belongings including items stored in outside storage house and medication room, and complete discharge paperwork.
- Upon discharge, strip consumer's bed on the day of discharge, sanitize mattress and pillow, and make-up bed.
- Serve meals, chart percentage of food consumed.
- Restock / organize supplies in kitchen pantry.
- Document water, refrigerator, and freezer temperatures in the kitchen, male and female bathrooms.
- Perform general cleaning duties as needed, including but not limited to, cleaning and restocking bathrooms, kitchens, rooms, etc.
- Assist consumer with washing / drying their clothing items.
- Check each shift to ensure all documents and inventory are labeled with each consumer's label.
- Check all documents prior to leaving for completion.
- Report to work on time and do not leave until relief has arrived and report given.
- Conduct monthly safety inspections and complete documentation.
- Report to supervisor when items require reordering.
- Clean transport containers and van daily.
- Conduct safety drills as scheduled and complete documentation.
- Other duties as assigned and / or requested by administrative and nursing staff.

Day Shift : 6am-6pm
Night Shift : 6pm-6am (<
Alternating weekends, 24 / 7 schedule, includes holidays.
