- Assist individuals and their families' receiving services and assist in coordination and receipt of supports. Such linking family with specialists outside of the agency or with a primary care physician. Also helping families or individuals with DD waivers.
- As a part of care work No show reports to get clients rescheduled for services. Make and document reasonable attempts concerning conversations, attempts to call, and mailing letters to engage clients and get them back into services.
- Will work with nurses, doctors, and clinical providers to ensure each client services have an official MD diagnosis.
- Coordinating and supporting transitions of care that include tracking of admission and discharge and coordination of specific services through completing baseline, reassessments, and discharge NOMS.
- Communicating and sharing information, including the transfer of medical records, information about appoints, services/treatment, and information on medications within care teams and other care coordination partners, as allowable and agreed upon with the individual being served.
Here are some of the things we require:
- High School Diploma/GED Equivalent (All Bachelor level and unlicensed care coordinators must be supervised by a licensed mental health professional.)
- Minimum of two (2) years clinical intervention experience in serving youth with SED or emerging adults with a history of mental health
- Valid Georgia Driver’s License
- Effective verbal and written communication skills
- Strong interpersonal skills and the ability to work effectively with diverse communities
- Ability to work independently and in collaboration with others
- Experience with Microsoft 365 Office Products
- Ability to organize, prioritize and meet deadlines accordingly
Here’s what would put you over the top:
- Bachelor’s Degree in a related field (Social Work, Psychology, Human Services, Etc.)
- Previous experience with Direct Care Coordination
