LOCATION: Swainsboro, GA
The Community Service Board of Middle Georgia's Braswell House is seeking a qualified individual to serve as our next Client Support Worker. This person will be responsible for providing care and assistance to adults with diagnosed intellectual and developmental disabilities, ensuring that they have the proper care needed throughout the day and in everyday activities. The selected candidate will also be responsible for keeping up to date progress notes on the individuals they work with, to ensure that the program manager has the proper knowledge to advise families and senior leadership on necessary next steps.
Responsibilities of the Client Support Worker:
- Providing basic needs for individuals and their families, such as helping with household tasks, personal care and paperwork
- Supporting and monitoring their healthcare needs, including administering medication and temperature checks
- Observes and monitors residents' behavior during assigned shift and properly charts observations and incidents in accordance with program policies and procedures in the EMR and logbook
- Make sure individuals exercise and walk according to weekly schedule
- Assist resident with personal hygiene/self-care skills
- Maintains a clean, healthy, and safe home environment.
- Assists resident in the completion of daily housekeeping chores, such as sanitizing bathrooms, cleaning refrigerator, and dusting furnishings, sweep, and mopping
- Reports all facility and equipment problems to supervisor while on shift or leave message
- Adheres to all state and federal laws and departmental rules and regulations with regards to semi-independent safety.
- Records 100% accurate time and dosage of all medications taken by residents using SSAMS protocol and policy.
- Completes incident and accident reports, when necessary, with established time frames and reports any incidents to the appropriate staff.
- Completes shift change report in houseparent logbook at the end of each assigned shift according to standard procedures.
- Completes urinary drug screens as needed according to procedure and documents results. If urine is to be sent to lab, stores urine properly.
- Display professionalism when interacting with individuals, the general public, and peers.
- Treats residents with respect, courtesy, and tact; listens to individuals and interacts with individual as a person while maintain a business relationship.
- Provides clear, accurate information; explains procedures and provides supplemental information.
- Communicates regularly with other shifts to facilitate effective care of individuals.
- Refrains from gossip or unprofessional communication regarding individuals or co-workers. Maintain healthy professional boundaries with all individuals.
- Does not receive from or give gifts to individuals.
- Provides transportation for residents to appointment and activities.
- Maintain a thorough knowledge of program policies and procedures and attend scheduled/mandatory meetings
- Attend in-service training as required.

- High School Diploma/GED Equivalent
- Valid Georgia Driver’s License
- Effective verbal and written communication skills
- Strong interpersonal skills and the ability to work effectively with diverse communities
- Ability to work independently and in collaboration with others
- Experience with Microsoft 365 Office Products
- Ability to organize, prioritize and meet deadlines accordingly

- Previous experience being a personal care assistant or certified nursing assistant
