The Certified Dietitian/Nutritionist will be primarily responsible for providing nutrition counseling and education to Medicaid recipients, either individually or in group settings. This includes assessing clients’ nutritional needs and food patterns, planning and directing the provision of appropriate food to meet physical and nutritional requirements and offering tailored nutrition counseling. The role also involves educating clients on meal preparation, grocery shopping, and maintaining a balanced diet to improve food security. The Certified Dietitian/Nutritionist will manage the organization’s My25 Nutrition and Wellness Program and collaborate with an interdisciplinary team to ensure that dietary plans are both effective and adhered to. The position requires in-depth knowledge of nutrition services, particularly those related to the 1115 Waiver or similar programs aimed at supporting food security and meeting nutritional needs. Additionally, a strong understanding of dietary guidelines, food safety, and managing health conditions such as diabetes, hypertension, and obesity is essential to the role.


POSITION RESPONSIBILITIES

  • Conducts outreach within 5 business days of receiving referral.
  • Updates the Inbound Referral Dashboard referral status for reach new referral.
  • Initiates service within 7 business days of receiving referral.
  • Provide nutrition counseling and education services, focusing on topics such as healthy meal preparation and grocery budgeting.
  • Assess the nutritional needs of individuals based on age, activity level, and special medical conditions (e.g., diabetes, high blood pressure, food allergies, obesity).
  • Develop and direct nutrition plans to ensure food provision meets physical and dietary needs.
  • Tailor nutrition counseling based on individual circumstances, including advice on dietary changes.
  • Plan menus and guide the preparation of food to meet dietary needs.
  • Conduct assessments of nutritional needs and food patterns, both individually and in group settings.
  • Provide education on meal preparation and grocery shopping to improve food security and nutritional habits.
  • Ensure that all services comply with best practice guidelines and food safety standards.
  • Collaborate with interdisciplinary teams to ensure effective delivery of nutrition services.
  • Educate clients on the importance of food safety in meal preparation and shopping. Sources and procures groceries and other items as needed to deliver culinary/wellness learning activities and events.
  • Work as part of an Interdisciplinary team to determine the nutritional needs of the individual.
  • Makes recommendations to the residential treatment team for the maintenance and/or improvement of nutritional status.
  • Manages the My25 Nutrition and Wellness Program. Recommend changes to meal plans to address nutritional concern.
  • Provides training to individuals and staff on dietary restrictions and choking risks.
  • Conducts periodic on-site observations to ensure dietary restrictions are being adhered to.
  • Monitors hygiene, sanitation and participant safety within kitchen environments (maintaining compliance with OSHA requirements as necessary).
  • Collaborates with the SCN Lead Entity on data-driven performance reporting in response to any regulatory reporting obligations.
  • Reports all Member incidents, compliance concerns, grievances or complaints using the Integrated Cared Incident Report.
  • Reports any suspected child abuse or neglect.
  • Completes all required documentation and billing requirements for service delivery.

Education and Experience

  • A New York Certified dietitian/nutritionists OR Registered Nurse Dietitian
  • Bachelor's degree in dietetics/nutrition with a minimum of six months of acceptable experience OR an associate's degree in dietetics/nutrition with at least eight years of acceptable experience.
  • Must provide proof of having passed relevant NY State-approved licensing examination.

Skills, Knowledge, and Abilities

  • Substantial knowledge of principles, methods, and procedures of the nutrition services covered under the 1115 waiver, or comparable services mean to support an individual in obtaining food security and meeting their nutritional needs.
  • The ability to plan, develop and adapt curricula/lessons for learners of all abilities.
  • The ability to adapt teaching styles and differentiate instruction to meet the diverse needs of learners.
  • The ability to evaluate learners’ progress in developing proficiency in culinary techniques.
  • The ability to organize, plan and manage classrooms with flexibility, dependability, discretion, and independent judgment.
  • Knowledge of the appropriate use of commercial kitchen appliances.
  • Capable of organizing information and self-monitoring completion of responsibilities
  • Good oral and written communication skills.
  • Comprehensive knowledge of mathematical principles and computer programs such as Outlook, Microsoft Word and Microsoft Excel.
  • Working knowledge of community-based services for the population.
  • The ability to read and write case notes/reports as well as understand written directions.

Certifications

  • Completion of assigned trainings through WNYICC
  • Must hold and maintain NYS certification for Dietitian/Nutritionist or Registered Nurse Dietitian
  • Must be or become, and maintain, ServSafe Manager Certification within 120 day introductory period.
  • Must be or become, and maintain, CPR/First Aid certification within 120 day introductory period.
