What are the responsibilities and job description for the ATI Court Liaison position at Community Services for Every1?
Alternative to Incarceration Court Liaison provides advocacy and acts as a liaison for Developmentally Disabled Individuals or individuals with serious mental health concerns who are involved in the Criminal Justice System. Works in partnership with ATI Assessor and the Court System /Probation to ensure information is communicated thoroughly and understood. Determines service system referrals and eligibility for treatment plan interventions, in working toward increased stabilization and reducing recidivism amongst enrollees. Provides individual and co-facilitates group Cognitive Based Intervention.
POSITION RESPONSIBILITIES:
- Provides services to people that are involved in the Criminal Justice Court System. Acts as a liaison between the Care Coordinator and/or Health Home Care Manager and the Court System to ensure information is communicated thoroughly. Assesses potential referrals and conducts initial admission meetings for those involved in the court process.
- Acts as a liaison between the Service Planning Team, PC of Criminal Justice Programs, and the court, communicating regularly between them to ensure information is conveyed clearly and reliably.
- Meets with individuals who are appearing in Court and guides them through the legal process, explaining each court proceeding and the outcomes.
- Acts as a liaison between the individual and the District Attorney, Defense Attorneys, Judge, Probation/Parole Officers, Police Department, and any other Criminal Justice Representative while they are involved in the court process.
- Monitor and track court attendance to report to the Service Planning Team.
- Prepare and present written reports to the court on compliance to services, progress and recommendations.
- Identifies problems that prevent people from fulfilling a court order and works with the PC of Criminal Justice Programs to resolve or recommend additional services.
- Provides intensive case management services including requisite advocacy/communication on needs, linkages, and progress.
- Provides crisis intervention as needed.
- Maintains contact with people receiving services and service providers, as needed, to ensure the needs of each person are met in a consistent, ethical, and effective manner.
- Attends all regularly scheduled Treatment Team meetings to maximize communication and knowledge of progress and concerns.
- Participates in program outreach and education to community members and other service providers.
- Maintains required records including case notes in compliance with funding source documentation requirements.
- Participates in specific staff meetings, training, and supervision.
- Communicates information that requires an OPWDD event to the Care Coordinator to ensure compliance with Agency and Quality Assurance standards.
- Demonstrates understanding and proficiency in Therap and the Integrated Justice Portal.
- Maintains NCIC certification biennially; and 10 hours of professional developmental training annually.
- Co-facilitates group Cognitive Behavioral Intervention.
- Provides individual Cognitive Behavioral Intervention and Interactive Journaling.
- Completes other duties assigned.
