What are the responsibilities and job description for the Job Coach position at Community Services for Every1?
Evaluates individual’s needs in relation to employment, seeks job placements, facilitates the job development process, communicates with businesses, and instructs and/or provides supports to assigned job seekers both individually and in group settings. Provides on-site job training with community placements.
- Conducts initial assessments of assigned individuals to identify their vocational skills, barriers, and preferences.
- Participates in planning of services and monitoring of employment goals for assigned job seekers on caseload.
- Coordinates services with ACCES-VR, OPWDD, Pre-ETS and other oversight bodies, service providers, and interested parties to monitor and ensure the highest quality of services are being provided to each individual.
- Assists job seekers with developing resumes, completing employment applications, attending job interviews, and other hiring requirements.
- Reviews employment ads and accesses other employment resources, such as attending job fairs and other community employment events.
- Identifies suitable employment for individuals based on choice, needs, preferences and individual abilities.
- Assists job seekers with securing positions, attending provided orientation for new positions, and maintaining business relationships through employer contact.
- Coordinates situational assessments and on the job training as necessary to ensure that job seekers meet employment requirements.
- Provides individualized mobility training to and from the individual’s place of employment. Utilizes personal vehicle to attend all meetings and appointments as determined by assigned caseload.
- Provides routine monitoring of individuals receiving extended/long-term services.
- Teaches Workplace Readiness, Job Exploration, and Work-based Learning classes as needed.
- Completes task analyses, placement forms, turnaround documents, consent forms, and other individual intake forms as assigned.
- Completes incident reports, appropriate service documentation, and records as required and in a thorough and timely manner to ensure compliance. Maintains and files records as necessary.
- Completes required ACCES-VR and OPWDD documentation within recommended time frames.
- Completes data entry for Therap and assists with maintaining the NYESS database to ensure individual files are updated and current.
