The Program Manager provides program oversight and staff supervision for the Community Habilitation and Dixon Respite Programs.
POSITION RESPONSIBILITIES
- Acts as a Leader and mentor within the department. Serves as a role model in Living the Mission. Displays ethical decision making.
- Models and promotes Person Centered Planning, considering the dreams and goals of individuals being served. Proactively plans to ensure active treatment of all served.
- Hires, orientates, trains, and supervises staff in all areas of job performance including ongoing assessment of service delivery as well as knowledge of environmental modifications pertaining to the site(s) for individual safety with oversight by the Program Coordinator.
- Provides staff training regarding behavioral plans and interventions, ensures completion of behavioral reports and ensures follow through with the Behavior Clinician.
- Coordinates scheduling and assignment of staff to ensure services are provided as well as the safety and minimum staffing pattern are always met. Works shifts as DSP as needed.
- Ensures assignment, completion, and documentation of required in-service training for Staff.
- Approves and reviews travel reimbursements for staff.
- Participates in, writes, and implements Community Habilitation Plans with oversight by the Program Coordinator.
- Oversees and ensures the staff implementation of plans and documentation of daily notes and monthlies are completed correctly. Reviews for accuracy and ensures all required elements are met.
- Completes data entry into Therap and assists with maintaining the database to ensure individual files are updated and current.
- Manages expenditures of petty cash and individual personal funds.
- Ensures provision of or acquisition of food, supplies and household items.
- Provides supervision and assistance to individuals to ensure their health and safety including personal care, dispensing of medications, and dietary needs.
- Implements SCIP-R when necessary.
- Develops, coordinates, and assists with programmatic activities. Locates community-based events to promote integration. Encourages program participants to be good neighbors, responsible citizens, join local clubs and support their community. Ensures participants satisfaction and input occurs with program activities, curriculum, and goals.
- Uses knowledge of NFTA system and encourages mobility training where appropriate.
- Maintains contact with Care Coordinators and families to ensure satisfaction with services. Follows up on concerns.
- Develops and maintains positive relationships with schools and community resources to enhance the development of the Program.
- Participates in DDAWNY Respite committee meetings and all other internal and external meetings as assigned.
- Ensures the ongoing safety of all staff and individuals by maintaining a safe environment as well as reporting any concerns whether through staff incident reports, safety plans, emails etc.
- Ensures protection and adequate care of individuals through review of Behavioral Incident Reports and Agency Incident Reports.
- Receives and reviews minor, reportable, and serious reportable incident reports, and follows up with Program Coordinator for direction and processing.
- Dispenses medications to individuals as prescribed. Coordinates with the pharmacy, Care Coordinators, families, nurses and doctors to ensure adequate supplies of medications are readily available.
- Works with Clinical Staff to ensure implementation and documentation of medical and behavior plans. Assists Behavior Clinician with ensuring behavioral plans/protocols are complete and meet the needs of individuals.
- Ensures compliance with all OPWDD quality and certification requirements as well as all fiscal requirements.
- Ensures routine maintenance and repairs of the Site and Site vehicle.
- Coordinates and schedules fire drills as well as all fire safety and mag lock checks at site.
- Ensure service documentation is submitted in time for billing. Completes monthly attestation and forwards it to Program Coordinator. Completes Monthly Analysis for the programs.
- Monitors and manages units of service to achieve objectives for HCBS funding source, as well as family utilization of services to ensure they are staying within the time allotted to them.
SUPERVISORY RESPONSIBILITIES
Supervises the full-time, part-time and Relief Direct Support Professionals (DSPs) assigned to their programs and any DSPs who are assigned to cover shifts.
